The Dementia Outreach Service (DOS) provides an outreach service across the Northern NSW Local Health District.
We provide education, case management and advocacy for people with a diagnosis of Dementia and their carers.
To be eligible to access The Dementia Outreach Service, clients are required to have a formal diagnosis of dementia. In some circumstances the Dementia Outreach Service can facilitate the assessment and diagnosis process.
The Dementia Outreach Service aims to support people with Dementia and their carers by providing education and knowledge, social support and continued community engagement opportunities. We help clients link in with other appropriate supports and services as their circumstances change.
DOS is a Commonwealth funded aged care service. DOS also sees younger people with dementia.
How can I access this service?
Individuals, family members, carers and Aged Care Service providers can contact My Aged Care on 1800 200 422 to request an assessment from the Regional Assessment Service (RAS). RAS will refer clients to DOS for assessment and management. https://www.myagedcare.gov.au/
