The Aged Care Assessment Team (ACAT) assesses the needs of frail older people. We see inpatients in private and public hospitals, as well as people living in the community, to support them in accessing care. ACAT staff can assist with determining a persons’ eligibility for Government subsidised Aged Care Services, including Community Home Care Packages or Residential Aged Care services.

ACAT includes Registered Nurses, Clinical Nurse Consultants, Occupational Therapists, Physiotherapists, Speech Pathologists and Social Workers. ACAT works with doctors and nurses who specialise in the care of older people, such as Geriatricians, Psycho-geriatricians and Nurse Practitioners.
