The Aged Care Assessment Team (ACAT) assesses the needs of frail older people. We see inpatients in private and public hospitals, as well as people living in the community, to support them in accessing care.

Our ACAT staff can assist with determining a persons’ eligibility for Government subsidised Aged Care Services, including Community Home Care Packages or Residential Aged Care services.

ACAT includes Registered Nurses, Clinical Nurse Consultants, Occupational Therapists, Physiotherapists, Speech Pathologists and Social Workers. ACAT works with doctors and nurses who specialise in the care of older people, such as Geriatricians, Psychogeriatricians and Nurse Practitioners.

Regional Assessment Service

The Regional Assessment Service (RAS) staff assess older people for Commonwealth funded, entry- level aged care services. These services are designed to support older people to live independently at home in their community. Our staff can assess people for a variety of services, for example a meal service, community transport, personal care, social support or nursing.

Dementia Outreach Service

The Dementia Outreach Service (DOS) provides an outreach service across the Northern NSW Local Health District. We provide education, case management and advocacy for people with a diagnosis of Dementia and their carers.

To be eligible to access The Dementia Outreach Service, clients are required to have a formal diagnosis of dementia. In some circumstances the Dementia Outreach Service can facilitate the assessment and diagnosis process.

The Dementia Outreach Service aims to support people with Dementia and their carers by providing education and knowledge, social support and continued community engagement opportunities. We help clients link in with other appropriate supports and services as their circumstances change.

DOS is a Commonwealth funded aged care service. DOS also sees younger people with dementia.

RAS will refer clients to DOS for assessment and management.
