The Cardiac Rehabilitation program involves a team of health professionals working to help you manage your heart condition. We provide individual cardiac assessment and education focused on reducing cardiac disease risk factors, monitoring and managing your condition and improving your health.
Our program is managed by the Cardiac Liaison Nurse in conjunction with our Physiotherapy Department and Exercise Physiologist.
The program is suitable for most people living with or at risk of heart disease and those recovering from medical or surgical intervention.
We provide a service to patients with a progressive illness, not exclusively cancer patients, to help maintain the best possible quality of life.
Registered Nurses and our multi-disciplinary team including social workers and cancer liaison staff, help patients monitor pain symptoms and offer support in collaboration with the patient’s General Practitioner and treating specialists.
Our Breast Care Nursing Service provides support, information and education to clients and their families where there has been a diagnosis of early breast cancer, with ongoing contact during treatment.
The Breast Care Nurse can help you navigate the breast cancer journey, linking you with relevant services in your local community.
We accept referrals from individuals, GPs, Specialists, BreastScreen NSW, BreastScreen Qld and other health professionals.
