The Transition Care Program offers goal-oriented, time-limited and therapy-focused care to help older people regain independence after a hospital stay. It provides older people with a package of services that includes low intensity therapy such as physiotherapy and occupational therapy, as well as social work, nursing support or personal care to maintain and improve physical or cognitive functioning.

The program aims to help older people return home after a hospital stay, rather than prematurely enter residential aged care.

Transition care is for older people who may benefit from more time and support to make a decision on their long term aged care options after completing their hospital stay.

The program accepts clients aged 65 years and over, or Aboriginal and Torres Strait Islander clients aged 50 years and over, or clients who have an age related illness.

Clients must have the potential to improve and achieve their stated goals in a community environment, and be medically stable.

Clients required a current Aged Care Assessment Team (ACAT) approval, which can be organised in hospital.
