The Chronic Care for Aboriginal People (CCAP) framework aims to provide State-wide and local chronic disease initiatives for Aboriginal and non-Aboriginal staff working with Aboriginal communities.

The program provides a practical approach to improving health outcomes for Aboriginal people living with, or at risk of developing, a chronic disease.

Services offered include home visits, monitoring an individual client’s health, assistance to access other services, building relationships between the client and GP, and providing clients with health education to learn how to stay well.

To be eligible for the program, people must identify as Aboriginal or Torres Strait Islander, be 15 years or older, and have Diabetes, or Chronic Kidney, Lung or Heart disease.
