It is crucial the NNSWLHD has the correct GP names to ensure that Discharge Summaries and Admission/Discharge Notifications (ADNs) are delivered correctly.

As the Australian Digitial Health Agency’s Service Registration Assistant Proof of Concept has been discontinued before a national solution is ready, the form below is now the way GP practice managers can update the LHD about GP movements until the national SRA solution is ready.
