The Tweed Heart Failure Program involves a team of health professionals working to help you manage your heart condition. We provide individual cardiac assessment and education focused on reducing cardiac disease risk factors, monitoring and managing your condition and improving your health.

Our program is managed by the Heart Failure Nurse in conjunction with our Physiotherapy Department and Exercise Physiologist.

Anyone aged over 18 can access this service. No referral is required. Our cardiac nurse will liaise with your GP prior to commencing the program to ensure there are no risks associated with exercise.

Hours: The Heart Failure Nurse is available between 8.30am - 4.30pm (Mon-Thurs)
