The Dementia Service provides early identification and diagnosis of dementia, which includes a formal memory assessment.

Through the Memory Assessment Program (MAP), our Clinical Nurse Consultant will conduct a comprehensive bio-psycho-social assessment.

Patients will need a referral from their GP if ongoing consultation with a specialist Psycho-Geriatrician or Geriatrician is required for a formal diagnosis of dementia or other conditions. We will provide the findings of our assessment back to the GP.

The Dementia Service also offers an informal risk assessment clinic, also known as a ‘Quick tick memory check’, for people who are concerned about changes in their memory, mood or behaviours. Please phone to make an appointment with the Clinical Nurse Consultant.

We can provide recommendations for a re-assessment schedule, Mild Cognitive Impairment (MCI), or start the process for MAP in consultation with the patient’s GP.

We also offer future aged-care planning and discussions, and support in linking to appropriate services.

Anyone with concerns about changes in their memory, mood or behaviour can access this service.
