Our community occupational therapist provides home assessment and intervention with the aim of helping people stay as independent as possible, and safe within their own homes.

Our services include home modifications, equipment prescription, and education about falls prevention, energy conservation, and pressure injury prevention and management.

We accept referrals from GPs, allied health professionals, community service providers, family and friends with the client’s consent, and self-referral.  

This service is available to people aged over 18 who live within the service area and who do not receive other funding for occupational therapist assessment (ie NDIS).
