A range of Lismore Community Health outpatient clinics and home-based services are provided from the Carroll Centre at St Vincent’s Hospital, Lismore.

Amputee Clinic    

The Amputee Clinic provides services to assess and manage adults and children who have an upper or lower limb amputation or limb deficiency.

We provide lifelong follow-up by a specialist multidisciplinary team to provide appropriate artificial limbs or devices, and manage health and pain issues related to a client’s amputation or limb deficiency.

Hours of Service: The clinic operates on alternate Wednesdays, 11.00am – 4.30pm.

Community Neuropsychology

The Community Neuropsychology service provides neuropsychological assessment of a client’s brain function.  

Neuropsychological assessment involves testing aspects of thinking, such as memory, concentration, speed of thinking, and reasoning abilities. It can show strengths and weaknesses in how well some areas of the brain are working.  This can assist in diagnosis, if it shows a particular pattern of difficulty.

The assessment can provide information that may help in developing strategies to manage conditions, for example memory problems.

Clients can gain a better understanding of the changes caused by their brain condition. Where there is a need to track progress over time, either improving or deteriorating, neuropsychology offers an objective way of measuring this.

ACCESS CRITERIA:

Community clients aged 18 years or over, with known or suspected acquired brain pathology causing changes to thought or behaviour.  For example, suspected dementia, CVA, brain tumour, encephalitis and other neurological conditions.

Excludes: long standing conditions such as autism, developmental intellectual disability, learning disability.  Referrals can be made by GP, medical specialist or allied health professionals. We do not accept referrals that are primarily requested to establish eligibility for NDIS, Centrelink or Driving, or referrals for medico-legal matters. Please contact a private provider in this instance.

 

Day Therapy Service

The Day Therapy Service provides centre based community rehabilitation services to adults through a multidisciplinary rehabilitation program. This service is free.

We accept patients from across the Lismore, Ballina, Richmond Valley and Kyogle (including Urbenville) council areas.

 

OUR ALLIED HEALTH TEAM INCLUDES:

Physiotherapy: aims to assist clients to improve or maintain their mobility, strength, balance and flexibility in order to reduce their risk of falls and improve their physical functioning.

Occupational therapy: aims to promote optimum participation in life circumstances, whether in the home environment or in the community.

Speech Pathology: provides clients with assessment and treatment for swallowing and communication difficulties.

Social Work: provides psychosocial assessments of clients in collaboration with the team and also provides liaison and referral to community services and agencies on behalf of the client and their family/carers.

Allied Health Assistants: Provide support to clinical staff and also supervise clients completing their therapy programs.

Intake Officer: Provides a single point of contact for all community based rehabilitation referrals and assists in the provision of an integrated multidisciplinary day therapy service.

ACCESS CRITERIA:

We accept referrals from hospital or community, GPs, Specialist and other Allied Health Services Clients presenting with difficulties in two of the following functional areas:

  • Mobility (including risk of falls)
  • Activities of Daily Living e.g. feeding, personal care, domestic duties;
  • Home Safety
  • Psychosocial/emotional Wellbeing
  • Communication
  • Swallowing
  • Employment/Recreation/ Leisure

Home Based Rehabilitation Service

The Home Based Rehabilitation Service is a multidisciplinary home visiting service for adults aged over 16. The service uses a case coordination approach to provide assessment and targeted therapy for people who require rehabilitation services within a home environment.

Therapy will be based on working towards the individual client’s potential to achieve their rehabilitation goals.  The team includes Physiotherapy, Occupational Therapy, Speech Pathology and Social Work. This service is free and open Monday to Friday.

Referral received from inpatient hospital and rehabilitation units Adults aged 16 years and older Client must be medically stable with multifunctional needs, requiring input from a minimum of two disciplines.  Resides in the Lismore, Ballina, Richmond Valley and Kyogle (including Urbenville) council areas.

 

Lymphoedema Therapy Service

The Lismore Adult Lymphoedema Service treats clients who have a primary lymphoedema diagnosis or who have developed secondary lymphoedema as a result of cancer or cancer treatment.

The service offers education, treatment and ongoing support for eligible people diagnosed with lymphoedema within the Richmond Network.

We accept referrals from GPs, specialists or other health professionals.

Physiotherapy Outpatients

We provide assessment and treatment for musculoskeletal and neurological conditions to people of any age who may benefit from physiotherapy intervention.

This usually involves a comprehensive assessment and development of an individual treatment plan in consultation with the patient.

Most treatment plans involve education, self-management strategies and specific exercise programmes, as well as other physiotherapy techniques.
