Aged Care Assessment Team    

ACAT services

Community Nursing

Our community nurses provide a range of services including wound care, continence management, management of venous access devices, palliative care nursing, and referral to other care providers including allied health and the Aged Care Assessment Team.  

We accept referral from individuals, GPs, health professionals and hospitals.

Hours::  8.00-4.30 Mondy-Friday.

Dental Services (Oral Health)

The Grafton Dental Clinic provides free dental care for children aged up to 18 years, and adults who hold a Health Care Card or Pensioner Concession card and permanently reside in the Northern NSW Local Health District.

Hours:  Mon-Fri 8.00am – 4.30pm Monday - Friday

Palliative Care

The Palliative Care service supports patients with a progressive, life-limiting illness, to help maintain the best possible quality of life.

Our team helps patients monitor pain symptoms and offer support for end of life care in collaboration with the patient’s General Practitioner and treating specialists.

Our multi-disciplinary team includes Specialist Palliative Care Nurses, with the support of a Specialist Doctor, and social workers.

The service provides a specialist nursing consultancy service for symptom management to the community. We also work closely with GPs, Community Nurses and hospital staff to ensure continuity of care, and Silver Chain for end of life care.

Inpatient care is provided at Grafton Base Hospital and Maclean District Hospital and assistance from trained volunteers is available for in-home respite, equipment loan and transport.

Patients will need a referral from their General Practitioner.

Transitional Aged Care Service

The Clarence Valley Transitional Aged Care Service (CVTACS) helps older people recover at home, rather than staying in hospital longer.

It offers goal orientated, time-limited and therapy-focused care to support older people to regain independence after a hospital stay.

People leaving hospital need different amounts of time to recover. Our service can help with a package of services for up to 12 weeks.

We can visit clients at home to help with daily or health care needs, so clients can lead a more independent life at home.

Our team works with the client and their carer, as well as a range of care providers and services:

* GP

* Physiotherapists

* Occupational Therapists

* Nurses

* Speech Therapists

* Dieticians

* Cleaning and shopping services

Together we plan and assist with therapy and support, including:

* Personal care

* Home exercise and mobility program

* Home safety check and provision of short term loan equipment

* Help with household cleaning and shopping

Clients must be medically stable and have a current Aged Care Assessment Team (ACAT) approval prior to leaving hospital.

The program accepts clients aged 65 years and over, or Aboriginal and Torres Strait Islander clients aged 50 years and over, or clients who have age-related illness.

Clients must be willing to participate in the program, with the potential to improve and achieve their personal goals in the community environment.

Women's Health Clininc

Our Women’s Health Clinic provides a wide range of services for women aged 14 and over.

These include: Cervical Screening Test (which replaces the pap smear test), breast checks, pregnancy testing and counselling, postnatal checks, post termination checks, continence assessment and sexual health screening. Clients can also access IUD (intrauterine device) insertion and contraceptive implants through the Women’s Health Clinic.

We also provide information on any women’s health issues, eg menstruation, menopause, osteoporosis, contraception, relationships and lifestyle issues.

The Clinic is conducted by a Women’s Health Clinical Nurse Consultant, and a doctor is also linked to the service. The doctor provides consultations from the Grafton Clinic.

We also provide health education and group programs on Women’s Health issues. Please contact us for more information.

Individuals can self-refer, and we also accept referrals from GPs and other health providers.

Hours: 8.00am – 5.00pm Monday to Friday. Clinics are on Thursdays by appointment only.
