Our community nurses provide a range of services including wound care, continence management, management of venous access devices, palliative care nursing, and referral to other care providers including allied health and the Aged Care Assessment Team.

Individuals can self-refer or be referred by a health professional or organisation including GPs, Hospitals, Specialists, Support Services and family/friends/carers.

If you are referring on behalf of someone else, please ensure they are aware or the referral. A Medicare Card is required.
