This is a multidisciplinary clinic which provides follow up, education and care co-ordination for Aboriginal clients over the age of 15 who have Type 2 diabetes or are at risk of developing diabetes.

The service is open to anyone of Aboriginal descent, or who has direct familial connections with Aboriginal communities in the areas of Casino, Coraki, Box Ridge, Tabulam, Bonalbo, Urbenville, Muli Muli, Woodenbong and Kyogle.

The service is run in conjunction with Healthy North Coast. Clients will need a written referral from their GP.
