Our Emergency Departments are open 24 hours a day, seven days a week:

Specialist Nurses in the Chronic Disease Management (CDM) service work with clients who have one or more diagnosed chronic conditions, have recently been in hospital, or are at risk of going to hospital and need help to manage their condition and stay well at home.

CDM services can help with locating the right type of care, as well as coordinating this care for the individual client to ensure they are well supported in the community. A Shared Care Plan can be developed for clients in the program together with their doctor and other health care professionals.

This service is available to residents over the age of 16 who have one or more chronic conditions, are at risk of hospitalisation within the next 15 months and are likely to benefit from one or more interventions such as care coordination and self-management support.

Access to the program is by hospital admission or referral from GP practice or community health. We also accept self or carer referrals.
