Pottsville Health One Dementia Service Clinic

This service provides both a formal process for the early identification and diagnosis of dementia as well as an informal process for memory assessment. The Memory Assessment Program (MAP) consists of obtaining a comprehensive bio-psycho-social assessment, neuro-psychological screening if indicated, and referral to Psycho-Geriatrician/Geriatrician as indicated for formal diagnosis of dementia and or differentiation of depression or other conditions. The process commences with referral from G.P. and completes with findings and recommendations back to the G.P.

The informal risk assessment clinic “Quick tick memory Check” is for people who are concerned with changes in their memory, mood or behaviours. The outcomes can be recommendations for re-assessment in 6-12 months as in normal age related changes, M.C.I., or to start the process for MAP in consultation with their General Practitioner.

  • 8.30am – 4.30pm (Mon-Fri)
  • Anyone with concerns about changes in their memory, mood and/or behaviour can access this service with a referral from your GP
