How do I access my medical records?

NNSWLHD HRIPA Application Form

Access to Medical Records in NSW is regulated by the Health Records and Information Privacy Act NSW 2002 (HRIPA) and the NSW Health Privacy Manual. Health Information Services (HIS) is responsible for providing you with health information according to the Act.

To obtain a copy of your medical record you must:

  • Complete NNSWLHD Health Records and Information Privacy Act (HRIPA) Application Form and
  • Provide relevant supporting documentation; e.g. Photo ID as detailed on the HRIPA application form.

Return completed forms to NNSWLHD-AreaCorporateRecords-GIPA@health.nsw.gov.au they will then be directed to the relevant Health Information Services (Medical Records Department) to process.

You may apply for access to Medical Records under the Government Information (Public Access) Act 2009 (GIPA Act). However, the first step is to make a request for your information using the NNSWLHD HRIPA Application Form. We will make every effort to release your personal or health information.


Can I view my medical record?

If you wish to view your medical record, please contact Health Information Services (see telephone contact details below) to arrange an appointment. There is no fee to view your record however an application form and supporting documentation will be required to confirm your identity and authority to view the medical record.
