Lismore Community Health – Hospital in the HomeAlternative to hospital service that provides acute type care in the home.Referrals are made via hospital medical officers who accept clinical governance for the treatment to occur. HITH Patients are classified as inpatients. Treatments provided include Intravenous antibiotics, pre surgery treatments, VAC wound dressings, sub acute surgical care e.g. wounds, stomas and drains that require acute nursing.
- Any medical condition requiring further treatment and assessed as medically stable and appropriate to be managed in the home. Must have access to phone. Voluntary program – must consent to home treatment. Mobilise independently or be able to manage safely at home. Cognitively able to manage safely at home. Must live within geographic area or be able to provide own transport to access clinic for treatment. Must have an appropriate residence that is suitable for home visits and be socially and emotionally stable. No IV drug users or current documented alcohol abuse due to patient and staff safety issues
- Potential clients for the HITH service need to be assessed by the HITH RN to determine suitability for the program in line with service criteria including WHS issues, treatment required and medical governance arrangements
- Referral by inpatient facility, GP or Specialist
