Carroll Centre – Community Based Rehabilitation

The Community Based Rehabilitation Service is a multidisciplinary community (home) based rehabilitation service which uses a case coordination approach to providing comprehensive assessment and targeted therapy for people who require rehabilitation services within a home environment. Therapy is targeted and based on working towards the individual client’s potential to achieve their rehabilitation goals.

  • 8.00am – 4.30 (Mon-Fri).
  • Access Criteria/Age: Adults (16 years and older) and their carers who are identified, whilst inpatients of an acute or subacute hospital unit, as being ready for discharge to their home environment with continuing rehabilitation therapy. Must be medically stable and requires multidisciplinary therapy from a minimum of two disciplines of Physiotherapy, Occupational Therapy, Speech pathology and /or Social Work.
