Grafton Community Health Occupational Therapy – Adult
The Occupational Therapy Department provides the following services to adults with a focus on the impact of disease, disability or aging on the person’s ability to manage in their normal daily roles and functions:- inpatient services to Grafton Base Hospital including rehabilitation, home assessments, equipment provision and client education
- community based home assessments for safe management at home
- advise and recommendations on major and minor home modifications
- Equipment Loan Pool – loan of adaptive equipment for a period of 2 months
- Prescription of complex aids and equipment
- Education on energy conservation
- Education on falls management
- Outpatient rehabilitation is provided on a limited basis dependent on clinical need
- Referrals are accepted from all sources, including self-referrals and from family and carers (with the individual’s consent)
- Referrals are prioritised on the basis of clinical need and are placed on a waiting list pending service availability
