Casino Community Health Cardiac Rehabilitation and Heart Failure Program

The purpose of this program is to facilitate and shorten the period of recovery after an acute cardiac event. The program also hopes to promote a sustainable self-management strategy of ongoing prevention for patients who have suffered any form of cardiac event, including angina, myocardial infarction, re-vascularisation procedures and heart failure. This will include assessment, support, education and exercise.

  • 8.30am – 5.00pm (Mon-Fri).
  • People over the age of 18 years and living in the Casino district and surrounds with diagnosis of a cardiac condition
  • People who are considered at immediate or high risk of a cardiac event and requiring a lifestyle change
  • Referrals are accepted from a Cardiologists, Specialists, GPs, Nurses, and Allied Health from local and metropolitan hospitals, community based services and self-referral. The Cardiac Nurse will contact the client at home on a Wednesday or Thursday to arrange an initial visit
