Submitting an Application to the NCNSW HREC

New Ethics Applications:

Please contact the NCNSW HREC Executive Officer prior to the submission of your application and be aware thatcomplete applications only will be accepted for review. Incomplete applications will be returned to the researcher for re-submission.

Please also be aware that applications received after the submission closing date will be held over until the next available meeting.

New applications must include:

  • 1 x complete collated package – which consists of:
    • Final completed and signed (original signatures) copy of the NEAF / LNR NEAF saved in PDF format from the online forms site
    • Protocol or Research Proposal/Plan
    • Applicable Brochures / Manuals / Product Information
    • Participant Information Statement/Sheet/s
    • Participant Consent Form/s
    • Data Collection Tool/s
    • Advertisements/Flyers etc
    • Surveys / Questionnaires / Interview Schedules / Semi-structured Interview/Focus Group Questions
    • Other relevant information – as/if required
  • 18 complete collated packages (copies) of the above (making a total of 19 packages)
  • All packages should be printed double-sided, stapled or fold-back clipped together and 2 hole punched

The above documents and other relevant information PLUS the PDF version of the original completed NEAF / LNR NEAF – unsigned (as saved from the online forms site)  should also be forwarded in electronic format, via email to: EthicsNCNSW@ncahs.health.nsw.gov.au

Please note: All supporting documents must have the document identification including the version number and document date in the footer of each page and must be received by the Executive Officer  by the submission closing date.

IMPORTANT!  Please ensure you allow sufficient time for the delivery of your application - applications received after the submission closing date will be held over until the next available meeting. 

Following review of a New Ethics Application:

Formal correspondence will be forwarded to the project Co-ordinating or Principal Investigator, via email, advising of the outcome of the HREC review. NOTE: No hard copy letter will be issued.

Examples of advice include:

  • Further information and/or clarification required prior to further review of the application
    NOTE: an outline of the required information will be contained within the formal correspondence to assist you preparing a response to the HREC.
  • Approved
    NOTE: Projects cannot commence until after Research Governance (SSA) Authorisation has been granted.

 

Responses to HREC requests ie: Clarification, further information and/or amendments:

Please submit your response to the HREC in accordance with the advice received in the formal NCNSW HREC correspondence - noting that responses to the full HREC will be required to be received by the submission closing date.

Please Quote the NCNSW HREC Reference Number and Project Title in all correspondence to the NCNSW HREC.

Please ensure that all updated documents are provided in both tracked and clean documents ie: two (2) of each updated document must be provided.

All documents must also include the document identification and updated version number and date in the footer of every page.

 

All other submissions to the HREC:

Other submissions to the HREC should be forwarded in both hard copy and electronic formats ie: 1 complete Original collated package and 18 complete collated packages (copies) (making a total of 19 packages)

Examples of other submissions include:

  • Amendments to a previously approved project
  • Annual Progress and/or Completion Reports
  • SAE/SUSARs (local only)
  • Protocol Violations
  • Updates eg: Publications or other relevant information

Responses to the HREC, in regards further information and/or clarification and/or amendments to the application and/or supporting documentation, must be submitted in accordance with the advice received in the formal NCNSW HREC correspondence - noting that responses to the full HREC will be required to be received by the submission closing date.

NOTE: All updates to projects must be forwarded to the relevant Research Governance Officer for authorisation/acknowledgement prior to being implemented – together with the HREC approval/acknowledgement of same eg: Amendments to the study and/or supporting documentation, reports, publications etc.
