NSW Chronic Diseases Management ProgramEducation and support is provided by specialist registered nurses for people with chronic disease (such as heart disease, high blood pressure, Chronic Obstructive Pulmonary Disease [COPD] and/or diabetes). Care Coordination and telephone based Health Coaching is provided to help clients manage their chronic conditions. A Shared Care Plan can be developed for clients in the program together with their doctor and other health service providers.
- Available to NSW residents over age 16. Clients must have a principle diagnosis of one of the Chronic Obstructive Pulmonary Diseases – Diabetes, Heart Disease, Heart Failure or Hypertension.
- Access to the program is by Hospital admission, Cardiac, Respiratory or Diabetes services, GP’s referral, Practice nurses and Allied Health providers.Self or carer referrals are also accepted.
