Short term case management of people with permanent disability, and frail aged and their carers, where there are complexities impacting on the care situation, and as a consequence the care situation is at risk of breakdown, resulting in hospitalisation, homelessness or premature institutionalisation. Aim of service is to conduct a comprehensive assessment, and resource the situation by linking into ongoing service support, and facilitating environmental modifications.
